Daily checks to stay healthy. Weight, medicine, drinking, sodium, being active, swelling, breathing, and energy
Weight
– Make sure the patient has a scale at home.
– Write down the patient’s weight every day. (Save it inside the app)
– Weigh the patient each morning at the same time (after urination, before eating/drinking, the same amount of clothing)
– Call the doctor if the patient has gained 2 lbs in a day or 3-5 lbs in a week.
Sodium Intake
Salt is sodium chloride. It retains water in our bodies. By decreasing salt in your diet, you can help prevent fluid overload and lighten the stress on your heart
